加州駕照資料遭冒用：用 INV 35 向 DMV 申請詐欺紀錄審查
若加州駕照或身分證號碼被冒用，或 DMV 紀錄出現疑似冒名的事故、罰單，可從 DMV 官網下載 INV 35 詐欺審查表，說明發生經過及涉及的紀錄。若證件遺失或被盜，表格要求先向 DMV 申請補發，再提交這份報告。
填妥後，依表格指示把報告和相關證明寄往 dlfraud@dmv.ca.gov；可用證明包括警方報告、信件及相關交易文件。缺少佐證可能延遲回覆，DMV 亦明示並非每份報告都會展開調查。只向官方指定渠道提交資料，勿在公開論壇張貼駕照號碼或證件副本。本指南不表示事故或罰單已自動撤銷。

California licence identity misuse: request a DMV fraud-record review with INV 35
If a California licence or ID number is misused, or a DMV record contains a suspected impersonation-related accident or ticket, download the official INV 35 fraud-review form and describe the incident and affected record. If the card was lost or stolen, the form instructs you to obtain a replacement before submitting the report.
Send the completed form and relevant evidence to dlfraud@dmv.ca.gov as instructed. Evidence can include police reports, correspondence and transaction records. Missing support may delay a response, and DMV states that not every report is investigated. Use the official channel; do not post licence numbers or document copies on a public forum. Submission does not automatically cancel a ticket or accident record.
This guide uses the currently supplied INV 35 form marked June 2020. Its original online publication date is unconfirmed.
