If the card was lost or stolen, the form instructs you to obtain a replacement before submitting the report. Send the completed form and relevant evidence to dlfraud@dmv.ca.gov as instructed. Evidence can include police reports, correspondence and transaction records. Missing support may delay a response, and DMV states that not every report is investigated. Use the official channel; do not post licence numbers or document copies on a public forum. Submission does not automatically cancel a ticket or accident record.
