紐約新生兒親子假：分清通知雇主與向承保人交件
紐約符合資格的員工，可在孩子出生後首十二個月內申請有薪家庭假作親子陪伴，最多十二週，給付按薪金的百分之六十七計算並設上限。親子假須在出生後開始，不適用於產前病況；可以分段使用，但以完整工作日為單位。

New York newborn bonding leave: employer notice and carrier submission are separate
Eligible New York employees may take paid, job-protected leave to bond with a newborn during the first twelve months after birth. Benefits can cover up to twelve weeks at sixty-seven percent of pay, subject to a cap. Bonding leave starts after birth, not for prenatal conditions, and intermittent use must be in full-day increments.
Give the employer at least thirty days' notice when the timing is foreseeable, or notify them as soon as possible otherwise. Employer notice is not the insurance application. Prepare Forms PFL-1 and PFL-2 with the required birth or parentage evidence, complete the employer section through the official process, and submit to the employer's insurance carrier.
The completed package generally must reach the carrier within thirty days after leave starts to avoid losing benefits. Check the workplace poster or ask human resources; call 844-337-6303 if the carrier is unknown. Keep copies and submission evidence. An application is not approval, and these New York rules should not be substituted for another state's family-leave system.
