The former Sierra Family website currently redirects to Western Sierra Medical Clinic. New patients should use the current New Patients entry to preregister and wait for staff to review the information and discuss availability. Submitting a form does not reserve a first appointment.
The Nevada City page lists more than one clinic. Tyler Foote Clinic is at 15301 Tyler Foote Road, Nevada City, CA 95959; call 530-292-3478. Its listed services include medical, dental, chiropractic and behavioral care. Specify the service and street address when booking, since each department may operate at different times.
Western Sierra asks first-time patients to arrive at least fifteen minutes early with photo identification, insurance or Medi-Cal cards and current medication bottles. Bring a child's immunization record for pediatric care. Ask about costs, coverage and language arrangements. A winter 2024–25 closure notice remains on the site, while older Sierra Family PDFs show different hours. Neither should be treated as the current holiday or appointment schedule.
