Omni Family Health's new-patient page asks for photo identification and, where applicable, insurance cards, a medication list and immunization information. Download official registration forms to complete before the visit. Clinical forms distinguish adolescents aged twelve to seventeen from adults and seniors, so select the appropriate packet.
Call or text 866-707-6664 to confirm the actual clinic and service. The website provides a Central California location directory; its Bakersfield corporate-office address in the footer is not necessarily the place where an appointment will occur.
Omni accepts many insurers, including Medi-Cal and Medicare, but patients should still verify their particular plan and service. People without insurance or facing payment difficulties can ask about the Sliding Fee Discount Application and required income records. Applying does not automatically make every service free. The patient portal provides records, results and appointment requests. It is not an emergency-care channel, and a new-patient application does not promise an immediate opening.
