SoHum Community Clinic in Garberville: appointments and referral contacts
Southern Humboldt Community Clinic is SoHum Health’s rural clinic, accepting new and returning primary-care patients at every stage of life. It is at 509 Elm Street, Garberville, CA 95542, with Monday–Friday hours of 8:30 a.m.–5 p.m. Call 707-923-3921, extension 1221 for appointments or extension 1224 for referral questions.
Listed services include annual wellness visits, chronic-condition follow-up, routine screening, vaccination and referral coordination. The clinic also lists video or telephone visits, behavioral health and a visiting-nurse program for homebound patients. Staff should confirm access and individual suitability; this guide does not recommend a personal course of treatment.
The official contact page separately lists Jerold Phelps Community Hospital at 733 Cedar Street. Its round-the-clock service does not make the Elm Street clinic open all night.
