夏威夷非工傷暫時失能保障：TDI-45 的三部分及申請期限

Hawaii nonwork disability benefits: the three TDI-45 sections and filing deadline
Hawaii Temporary Disability Insurance partially replaces wages for qualifying nonwork-related illness or injury; it does not pay medical bills. Basic eligibility includes 14 Hawaii employment weeks with at least 20 paid hours per week and at least $400 total earnings in the preceding 52 weeks. Current-employment and medical-certification conditions also apply.
Notify the employer immediately and request TDI-45. Complete Part A, obtain the employer’s Part B and the medical provider’s Part C, then follow the filing route for the employer’s insured or self-insured plan. File within 90 days of disability onset to avoid possible benefit loss. Confirm payment and waiting-period rules under that plan rather than relying on older yearly dollar examples on the webpage.
