New York off-job disability benefits: route DB-450 according to employment status
New York disability benefits provide partial cash wage replacement for eligible workers unable to work because of an off-the-job illness or injury. They are not medical insurance paying treatment bills, and cannot be received at the same time as Paid Family Leave.
Use DB-450, normally within thirty calendar days of the first disability day; do not file before disability begins. Someone employed or becoming disabled within four weeks after employment ends generally submits to the employer or its carrier. After more than four weeks of unemployment, check the official unemployment and eligibility conditions to determine the Workers' Compensation Board route.
Keep your completed copy, ask the healthcare provider to complete Part B and the employer to complete Part C. Current instructions say an employer's delay should not make you wait indefinitely, but incomplete medical certification can delay payment. Use the official employer-coverage search to identify the carrier. Call 877-632-4996 for procedural or language assistance. Eligibility, payment and review of a rejection depend on the formal notices.
