紐約小企業 Healthy NY：向承保公司申請，先核對員工與供款條件

Healthy NY for small businesses: apply to the insurer after checking workforce conditions
Healthy NY is a DFS-supervised small-business group health program offered through insurers. It is not individual coverage for a person or sole proprietor. A business must operate in New York and satisfy requirements including one to fifty full-time-equivalent employees during the previous year.
Eligibility also considers the share of lower-paid employees, prior group coverage, employer payment of at least half the premium and employee participation. The website now presents the following year's wage threshold, which insurers may adopt early. Confirm the threshold for the intended coverage start date rather than relying on an older search result.
Choose an insurer serving the county and send the signed application and first month's premium directly to that company, not DFS. Annual recertification remains necessary. Check provider networks, personal costs and the start notice. Call DFS at 800-342-3736 for a paper application if needed. Submission is not immediate coverage.
