The Medical Board of California distinguishes requesting access to your own medical records from asking one clinician to transfer records to another. The patient-access rules explained on its page have defined conditions; the two requests should not be assumed to follow an identical process.
For personal copies, make a written request to the physician, specify whether you need all records or particular sections, and keep a copy. Reasonable identity verification and some copying or administrative costs may apply. The page describes response periods, but also explains provisions for summaries, minors and certain mental-health records.
