남부 LA 카운티 MSSP: 한국어 안내로 돌봄 지원 자격 확인하기
Independence at Home(IAH)의 MSSP는 집에서 생활을 이어 가도록 돕는 무료 사례관리 프로그램입니다. 현재 IAH 안내와 캘리포니아 노인국 자료는 60세 이상, 해당 Medi-Cal 자격, 요양시설 수준의 돌봄 필요 등을 조건으로 설명합니다. 나이와 보험 가입만으로 자동 승인되지는 않습니다.

MSSP in southern LA County: using Korean guidance to check eligibility
Independence at Home’s MSSP is a no-cost care-management program supporting life at home. Current IAH and California Department of Aging guidance describes age 60+, qualifying Medi-Cal eligibility and nursing-facility-level care needs; age and insurance alone do not establish eligibility.
IAH assesses assistance with at least two daily activities, such as bathing, dressing or mobility, and participation in case-management calls and visits. Check the official ZIP list for the approved area. Meals, transportation and home support depend on assessment and other available services.
Its Korean brochure offers free language assistance. Request Korean support and eligibility screening at (866) 421-1964 or communityoutreach@scanhealthplan.com. The Korean brochure states that these MSSP services are not provided to SCAN Health Plan members. Members should call (800) 559-3500 separately about their plan benefits.
