Upstate Family Health Center’s intake requires its application, consent and record-release documents, plus insurance proof and photo ID. Submitting the packet does not automatically establish active-patient status. Staff contact applicants after receiving and reviewing previous records to schedule the first visit and discuss any further care plan.
Mail the documents or deliver them during business hours to 1001 Noyes Street, Utica, NY 13502. Call 315-624-9470 for form or scheduling assistance. Treat website waiting-time estimates as estimates rather than reserved appointments.
