Locations in Buffalo, Niagara Falls, Lockport and Cheektowaga have different services and schedules, which should be confirmed separately.
Its Online Forms page provides new-patient registration, release authorizations and sliding-fee documents. Review them and ask what income or insurance evidence to bring. Downloading a form does not confirm an appointment or free care. CHCB also states that occupational therapy is being removed from its service scope; ask about referral help rather than confusing it with the physical therapy still listed.
