FEGLI 身故理賠：在職與退休身分決定通報及表格送交途徑
聯邦員工團體人壽保險 FEGLI 的身故申索，須先按死者身分選擇程序。在職員工或其受保家屬身故，通知僱用機關的人力資源辦公室；退休年金領取人身故，可向 OPM 網上通報或致電 1-888-767-6738。
受保本人身故使用 FE-6；已選購 Option C 的家屬保障使用 FE-6DEP。依相應指示附死亡證明認證副本，勿把所有表格寄到同一地址。遞交至少三十日後查理賠進度，可致電 OFEGLI 1-800-633-4542，備妥受保人資料及死亡日期；通報身故與獲准賠償是不同步驟。

FEGLI death claims: employment or retirement status determines the reporting route
FEGLI death claims begin with the deceased person’s status. For an active employee or an insured family member, report the death to the employing agency’s human-resources office. For a deceased annuitant, report to OPM online or call 1-888-767-6738.
Use FE-6 for the insured person’s death and FE-6DEP for family coverage under elected Option C. Follow the relevant instructions and include a certified death certificate; the forms do not all go to the same address. After at least thirty days from submission, ask OFEGLI about claim status at 1-800-633-4542, with insured-person details and the date of death ready. Reporting a death does not itself establish benefit entitlement.
