California facility-care complaints: identify the regulator and record the incident
CDPH receives noncompliance complaints about facilities it licenses or certifies, including hospitals, skilled nursing facilities and certain clinics. Patients, relatives or other members of the public may complain, including anonymously. Cal Health Find routes the normal online form to the responsible district office.
Prepare the facility name, incident date, care details and relevant attachments, and retain a printed copy before submission. A physician’s individual licensing issue may belong with the Medical Board, while nonmedical assisted living is overseen by Social Services. Check the official categories so the complaint reaches the appropriate regulator.
