紐約 CHN 新病人：電話登記、費用核對與青少年帳戶權限
Community Healthcare Network 在紐約多個區設診所。新病人或偏好電話預約者可打 866-246-8259；現有病人才使用官網列的 Healow 線上預約入口。先選合適分店和服務，不要把總部或另一分店地址當作看診位置。

Community Healthcare Network operates clinics across several New York City boroughs. New patients and anyone preferring telephone booking can call 866-246-8259; the listed Healow scheduler is for existing patients. Confirm the appropriate branch and service instead of using a headquarters or unrelated location.
For a first visit, arrive fifteen minutes early with identification, address and income evidence, medication information and previous health records. CHN describes income-based fees and access regardless of ability to pay, but laboratory work and some medicines may incur additional charges. Ask when booking. Give at least one day's notice to cancel, and update an insurer that requires a designated primary-care provider when changing doctors.
A notice effective October 1, 2026 limits parental Healow access for children twelve and older to immunizations and referrals. This does not erase records: ask the clinic about formal requests and applicable restrictions. Do not share passwords or publish medical documents on a forum.
