Concerned about a driver: California DMV safety referrals
California DMV provides form DS 699 for requesting a driver reexamination. A signed letter to a local Driver Safety office is another route. Identify the driver and describe specific incidents and concerns, rather than relying on age or a personal disagreement as evidence of declining ability.
The request must be signed; it is not an anonymous complaint. You may request confidentiality, which DMV handles to the extent permitted by law. Do not promise relatives that a reporting person's identity can never be disclosed. Medical information has separate confidentiality protections.
DMV may request medical documentation or arrange law, vision and driving assessments. A referral does not itself mean immediate licence revocation. Drivers should follow their actual notice instead of treating general online guidance as a decision on their case. Qualified interpretation can be requested, except during the driving test itself.
