加州工傷後的起點：通報僱主、索取申索表及保存副本
在加州工作受傷，或懷疑疾病是工作逐漸造成的，應盡快通知主管。工傷補償的用途包括合適醫療、部分工資損失及重返工作支援，並不是痛苦損害賠償；是否符合某項福利須按個案處理。
如有緊急情況，先打 911 或到急症室，並告訴醫療人員傷病與工作有關。非緊急情況也不要拖延通報。DWC 提醒，未在三十天內報告，可能失去取得福利的權利，不能把期限當成建議等待時間。
接著索取 DWC 1 申索表，按官方步驟填妥僱員部分、交回僱主並保留副本。僱主獲悉受傷後一般須在一個工作天內提供表格；沒有收到時，可從官方頁下載或致電 1-800-736-7401 查詢。官方亦提供中文表格及免費說明資源；不要只用一般病假申請代替工傷申索程序。

After a California workplace injury: reporting and the claim form
Notify your supervisor promptly after a California workplace injury, or when you believe a gradually developing illness was caused by work. Workers' compensation can provide medical treatment, partial wage replacement and return-to-work support. It is not compensation for pain and suffering, and individual benefits require assessment.
For an emergency, call 911 or go to an emergency room and explain that the condition is work-related. Do not delay reporting a nonemergency injury either. DWC warns that failing to report within thirty days could affect the right to benefits; that deadline is not a recommendation to wait.
Request the DWC 1 claim form, complete the employee section, return it and keep a copy. Employers generally must provide the form within one working day after learning of the injury. If it is missing, use the official download or call 1-800-736-7401. Chinese forms and free information resources are available. An ordinary sick-leave request does not replace this claim procedure.
