Valley Health Team offers family, pediatric and other care in California’s Central Valley. Call 559-693-2462 and confirm the appropriate location; it is separate from similarly named out-of-state Valley Health organizations.
Its first-visit checklist includes photo identification, any insurance card, immunization records, medications or a prescription list, medical history and address evidence. Uninsured people can bring a month of pay stubs or the previous year’s tax return for sliding-fee assessment. Ask about acceptable alternatives if documents are missing. An application is not a discount approval. New-patient forms are also available to review before the visit.
