看診前準備用藥清單：處方藥、成藥與補充品一起記錄
到不同診所、化驗室或藥房時，一份最新的用藥清單可減少反覆回想，也方便醫護人員了解你正在使用的產品。FDA 的指引不只包括處方藥，亦包括成藥、維他命及其他補充品。
可用紙張、手機內的清單，或官方頁面連結的 My Medicines 表格。逐項記下名稱、藥品強度、用途，以及目前依指示服用的份量、時間和方式；同時列出過敏資料及緊急聯絡人。若有不確定的項目，可拿藥袋或包裝向藥劑師核對，不必靠記憶補填。
當處方或實際用藥情況改變時，更新清單，保留一份隨身可取用的副本，並讓會陪同看診或協助溝通的可信任照顧者知道如何取得。這是紀錄工具，不是自行停藥、換藥或調整劑量的依據；用藥疑問應向醫護人員查詢。

Prepare a medication list: include prescriptions, nonprescription medicines and supplements
A current medication list can make visits to different clinics, laboratories and pharmacies easier. FDA's guidance includes prescription medicines, nonprescription products, vitamins and other supplements, rather than prescriptions alone.
Use paper, a list on your phone or the My Medicines form linked from the official page. For each item, record its name, strength, purpose and the amount, timing and method you currently follow. Include allergies and emergency contacts. If information is unclear, ask a pharmacist to check the packaging or medication label instead of filling gaps from memory.
Update the list when your actual medication use or prescription changes. Keep a copy that you can reach while away from home, and share access with a trusted caregiver who accompanies you or helps communicate during appointments. The list is a record, not instructions to stop, replace or change a medicine; discuss medication questions with a health professional.
