找不到疫苗紀錄？先查接種機構、舊文件與州登記系統
需要為入學、工作或旅行整理疫苗紀錄時，不要假設 CDC 能替你調出全部接種資料。CDC 說明，美國沒有保管所有個人疫苗紀錄的全國機構，CDC 本身亦不持有這些資料。
先查看家中保存的接種卡與舊文件，並詢問曾替你保管兒童接種資料的家人。之後可向以前的學校保健服務、可能要求接種紀錄的僱主，以及接種診所或公共衞生機構查詢。部分州的免疫接種資訊系統亦收錄成人紀錄；各機構保存年期及涵蓋資料未必相同，查不到不代表從未接種。

Missing vaccination records: where to start looking and how to keep a copy
When assembling vaccination records for school, work or travel, do not assume that CDC can retrieve a complete personal history. CDC explains that no national organization keeps everyone's vaccination records, and CDC itself does not hold them.
Start with saved vaccination cards and childhood papers, and ask relatives who may have kept your records. Then try former school health services, employers that required immunizations, and the clinics or public health providers that gave the vaccines. A state immunization information system may also contain adult records. Retention periods and coverage differ, so an unsuccessful search does not establish that a vaccination never happened.
Once you obtain a copy, store it safely with important documents. Bring the record to future vaccination visits and ask the provider to record, sign and date new entries. If records remain unavailable, discuss next steps with a health professional rather than guessing dates.
