New York’s ConnextCare assesses medical and dental sliding fees using household size and income. Insured patients may have part of their personal responsibility adjusted, but the official policy excludes insurance copayments. Some laboratory and radiology support for uninsured patients is subject to conditions, including ConnextCare-related care and services at Oswego Hospital.
Prepare household-income evidence such as the latest tax return, three recent pay stubs, or unemployment, public-assistance and Social Security income documentation. Call 315-298-6564, extension 2116, email outreach@connextcare.org, or ask a center for help. Confirm eligibility and the specific service first: sliding fees do not mean that every bill is waived.
