紐約工作受傷後：看診、通知僱主及 C-3 申索是不同步驟
在紐約因工作受傷，先盡快接受治療，告知醫護與工作有關及僱主資料。緊急情況可找任何醫護；非緊急情況應使用 Workers’ Compensation Board 認可提供者。盡快以書面通知僱主，一般須在受傷三十日內完成。
另向 Board 提交 Employee Claim C-3，可網上辦理或按表格地址郵寄；事故申索一般須在兩年內提出，不要只依賴僱主報告。若同一部位曾受傷或有相近疾病，另需 C-3.3。受理、醫療保障及工資損失補償均須按個案核定，這不是非工傷 DB-450 失能福利程序。

After a New York workplace injury: treatment, employer notice and a C-3 claim are separate steps
After a work-related injury in New York, obtain treatment promptly and tell the provider that it happened at work, including employer details. Emergency treatment can be with any provider; otherwise use a Workers’ Compensation Board-authorized provider. Notify the employer in writing as soon as possible, generally within thirty days of injury.
Separately submit Employee Claim C-3 online or by mail to the address on the form. Accident claims generally must be filed within two years; do not rely only on the employer’s report. A prior injury to the same body part or similar illness also requires C-3.3. Acceptance, medical coverage and lost-wage eligibility are determined individually. This is separate from off-job disability Form DB-450.
