Jericho Road uses separate contacts for medical and dental care. The Barton Street medical clinic is at 184 Barton Street, Buffalo, telephone 716-881-6191. Broadway dental care is on floor two at 1021 Broadway, telephone 716-348-3018. New patients should prepare registration forms, photo identification and an insurance card; uninsured patients can bring household-income evidence and ask about public coverage and sliding-fee assistance.
Medical arrivals more than fifteen minutes late and dental arrivals more than ten minutes late are rescheduled. Ask the clinic whether a follow-up is suitable for telehealth. An after-hours answering service, including interpretation, is available at 716-881-6191; this does not mean the clinic stays open around the clock.
