Medicare 居家護理：專業照護與全天家務照顧不能混為一談
Medicare 的 home health 保障針對符合條件的居家專業護理，不是全天陪護或一般家務服務。通常須需要間歇或部分時間的專業照護，並符合 homebound 條件；這不代表絕對不能為求醫或短暫活動離家。
開始前須由合適醫療人員面診、確認需要並下醫囑，再由 Medicare 認證機構提供服務。只需要洗澡、穿衣等日常協助，或單純送餐、清潔，一般不屬此項保障。家居護理助理的支援也有與其他合資格照護同時接受等條件。

Medicare home health: skilled care is not round-the-clock household assistance
Medicare home health covers eligible skilled care at home, not round-the-clock companionship or ordinary housekeeping. A person generally needs intermittent or part-time skilled services and meets homebound conditions. Homebound does not forbid every medical trip or brief outing.
An appropriate clinician must assess the person in person, confirm the need and order care delivered by a Medicare-certified agency. Personal assistance such as bathing alone, meal delivery and unrelated cleaning are not covered under this benefit. Home-health aide support also has conditions involving other qualifying care.
Covered home-health services have no charge, but durable medical equipment generally involves twenty percent of the approved amount after the Part B deductible. Ask the agency for written information about uncovered items and expected personal costs, including an ABN when applicable. Medicare Advantage members should check their own plan's arrangements. A recommendation does not establish coverage for every hour, item of equipment or household task.
