For community care in Pleasanton, Livermore or Dublin, call Axis Community Health at 925-462-1755. This is the California Tri-Valley organization, not the similarly named out-of-state Axis Health System. Dental callers can use option two. Specify whether the request concerns enrollment assistance or a medical visit.
Enrollment staff can assess which coverage or fee-assistance process fits the situation. Request the document checklist, including identification and relevant income records. For remote submission, use the encrypted link provided by staff; documents can also be delivered as instructed. Ask for translation when arranging the visit.
Medical services require an appointment. Enrollment walk-ins may be accepted, but that does not mean an immediate clinical visit is available.
