Preparing for AICHC care: interpreters, records and income-based discounts
All-Inclusive Community Health Center’s Burbank clinic is at 1311 N. San Fernando Blvd, Burbank, CA 91504; call 818-843-9900 to confirm services and appointments. Primary-care, psychiatric and pediatric televisits are listed, but remote care is not appropriate for every situation.
New-patient preparation includes photo ID, a medication list, relevant records from care in the past three years and an insurance-termination letter when applicable. Some programs require income evidence. Bring recent hospital or specialist records when transferring care. Sliding-fee discounts depend on household size and income; ask reception rather than assuming all treatment is free.
The center lists staff languages including English, Spanish, Armenian, Russian, Vietnamese and Farsi. Other-language users can request LanguageLine interpretation beforehand. A translated website menu is not a roster of clinicians’ languages. Existing patients can call 818-843-9900 during business hours and select 4 for records or 2 for medication/refill questions. Records-release requests may also be faxed to the Medical Records Department at 818-843-9901. A refill inquiry is not an approved prescription.
