California’s West County Health Centers asks new patients to schedule an establishing-care appointment before submitting new-patient paperwork. Registration is also updated annually. Some online form links are being rebuilt, so request missing forms from the office; completing paperwork is not confirmation of an appointment.
Uninsured patients seeking income-based sliding fees can upload income documentation or bring it to the visit as directed.
