BACH’s appointment number is 510-770-8040 for Alameda County and 408-729-9700 for Santa Clara County. State the desired clinic and service when calling; the Fremont administrative-office address is not the location for every appointment. The appointment line also offers after-hours contact.
For a first visit, bring photo identification, any insurance card, income evidence and current medications. Prepare proof of address, Social Security card and birth certificate where applicable; children should also bring immunization records. Registration forms can be downloaded beforehand. Income documents help assess reduced-cost services or insurance eligibility, rather than guaranteeing that every service is free.
