PACE 社區長者照護：四項資格要同時符合
PACE 把醫療與社會支援交由照護團隊統籌，協助部分長者繼續在社區生活。Medicare 官方列出四項基本條件：至少 55 歲、住在計劃服務區、由州確認需要護理院級別照護，並能在計劃協助下安全生活於社區。
先用官方入口查所在地是否有 PACE，再向計劃或州 Medicaid 辦公室確認資格、費用及照護安排。參加後，處方藥保障由 PACE 處理；自行另外參加 Medicare Part D 藥物計劃會導致退出 PACE。作出任何保障變更前，應先核對銜接安排。

PACE community care: check all four eligibility conditions
PACE coordinates medical and social support through a care team so some older adults can remain in the community. Medicare lists four conditions: age 55 or older, residence in a PACE service area, state certification of nursing-home-level care needs, and ability to live safely in the community with help.
Check local availability, then ask the program or state Medicaid office about eligibility, costs and care arrangements. PACE provides prescription-drug coverage for participants. Joining a separate Medicare Part D drug plan causes disenrollment from PACE, so confirm how coverage will connect before making a change.
