Davis Street Primary Care Clinic is at 3081 Teagarden Street, San Leandro, CA 94577, with clinic hours Monday–Friday, 8 a.m.–6 p.m. Published services include adult and pediatric care, dentistry and behavioral health, with in-person and telehealth appointments. Start at 510-347-4620 to confirm the service, appointment and appropriate extension. Different official pages show different extensions; this guide retains the consistent main number rather than treating them as one booking line.
The payment page says patients are treated regardless of ability to pay. Uninsured patients can apply for an income-based sliding fee; this does not make every service free.
