Clinica de Salud del Valle de Salinas（CSVS）服務 Salinas Valley 及 Monterey Bay 周邊，初診資料分成醫療、牙科及驗光三類，均設英語和西班牙語文件。官方要求新患者下載、列印及填妥相應表格，並帶往已安排的預約。

Salinas Valley CSVS first visits: choose medical, dental or optometry forms
Clinica de Salud del Valle de Salinas (CSVS) serves the Salinas Valley and greater Monterey Bay. Its first-visit documents are grouped into medical, dental and optometry services, with English and Spanish versions. New patients are asked to download, print and complete the relevant forms and bring them to their scheduled appointment.
Medical forms distinguish pediatric visits from the version for ages sixteen and over. Dental forms separate children and adults, while optometry provides an all-ages version. Choose by service and age; completing paperwork does not book an appointment. If the clinic or documents are unclear, contact the appropriate location through the official Find a Clinic directory before traveling.
