Prism Health North Texas recommends allowing about sixty minutes for a first visit. Bring photo identification, current prescriptions, previous medical records and residence and income information. Its guide specifically says to attend the appointment even if some or all documents are missing, so staff can help connect the patient to care.
Call 214-521-5191 or use the official appointment-request form; returning patients can also use the patient portal. Same-day and walk-in openings depend on availability, so call first. Ask about charges, insurance and financial assistance for the requested service rather than assuming an accepted request means all care is free.
