FDA 接收藥物、醫療器材及其他受管產品的安全問題通報。若出現不良反應，先聯絡醫護人員；MedWatch 是通報系統，並不提供診療建議。

Problems with medicines or medical products: use FDA’s reporting route
FDA receives safety reports about medicines, medical devices and other regulated products. Contact a healthcare professional first if an adverse reaction occurs; MedWatch is a reporting system, not a medical-advice service.
Start with FDA’s official “Report a problem” page and select the product category so the concern reaches the appropriate form. A clinician can help organize medication and test information, or consumers may report their own experience. Keep the product name, sequence of events and relevant records. Reports support safety monitoring, but submission itself does not establish causation or mean that FDA has decided to recall a product.
