Bring completed registration paperwork; additional history forms may still be needed at the visit. Filling out a form does not itself complete a consultation.
The official checklist includes identification, address evidence, insurance cards and relevant discharge or immunization records. For a sliding-fee assessment, ask which household-income documents are needed; the current form gives pay statements and W-2 forms as examples. Submit personal information only through proper clinic channels, not public comments.
LBU explicitly says services are not universally free. Insured patients may owe copayments, while uninsured fees depend on household size and income.
