Relevant grounds include medical necessity, experimental treatment and specified out-of-network issues. First identify the denial reason rather than treating every billing problem as the same appeal. DFS generally requires the application within four months of the final adverse decision on the internal appeal, or a waiver of that process. An optional second internal appeal does not restart that deadline. Keep the decision letter, dates and plan details, then check the patient-consent and physician forms required for the particular appeal. The official page provides online submission, paper forms and Chinese instructions. Eligibility screening requires complete documents. Fees, exemptions and refunds depend on the circumstances.
