Age, income, a diagnosis or inability to drive does not automatically establish eligibility; an individual assessment is required. Applicants complete a one-page Data Card and an approximately thirty-minute telephone interview. The official page provides materials in Chinese, English, Spanish and Vietnamese. Call 408-321-2381 about eligibility. Ask for alternative formats or language assistance when needed rather than assuming that a doctor must handle every step. Once eligible, reserve a trip one to three days ahead at 408-321-2380, between 8 a.m. and before 5 p.m.; a next-day journey must be arranged before 5 p.m. on the preceding day.
