Entry is generally within thirty days of discharge, into a Medicare-certified facility, with a documented need for daily skilled care. The hundred-day maximum per benefit period does not guarantee a hundred approved days. In 2026, days 21–100 carry $217 daily coinsurance; the initial period also involves the applicable Part A deductible. Some ACO arrangements or Medicare Advantage plans waive the three-day rule.
