The official page also includes injury or death involving an e-bike or e-scooter. Police attendance does not establish that the driver's own reporting obligation is finished. MV-104 gives a ten-day filing requirement. Injury or death also requires immediate police notification, a separate step from the DMV report. Property-only crashes still require the appropriate exchange of license, insurance and registration information. A crash appearing on each involved driver's record does not mean DMV has decided fault. Use the current online process through an NY.gov account, or complete the form and mail it to the address on its second page.
