Medicare requires a doctor's admission order and formal admission. Emergency care, observation or an overnight stay after outpatient surgery can remain outpatient services, affecting Part A, Part B and subsequent skilled-nursing coverage. Ask the doctor, social worker or patient advocate about your status each day. An expected need for medically necessary care across two midnights informs the decision, but two nights do not automatically create inpatient status. More than twenty-four hours of outpatient observation requires a Medicare Outpatient Observation Notice, or MOON, explaining the classification and possible costs. Keep notices and bills, ask whether the classification changed, and request information about applicable appeal rights. A limit on a single outpatient copayment does not necessarily cap all outpatient charges below the inpatient deductible.
