Notify police when both plates are missing, or when a vehicle issued only one plate loses it. For an incident within New York, law enforcement supplies MV-78B; it is not a form available for self-download from DMV. For an incident outside the state, obtain a report on that police agency's letterhead. If police refuse a report, follow DMV's MV-1441.3 instructions, including the date requested and the refusing agency. Do not invent a police report or incident details. Bring any remaining plate, the required report or statement, MV-82, identity evidence and insurance proof to DMV. The fee exception for theft or criminal destruction requires documentation describing that circumstance; ordinary loss is not automatically free. Confirm the plate category and office appointment process first. This guide files no report and promises neither retention of the old number nor exemption from every charge.
